POSITION SUMMARY

Works with patients, their families, guardians, physicians, health care
professionals and community resources, to provide medically-related social
services to attain or maintain the highest practicable physical, mental and
psychosocial well-being of each patient. Protect and advocate for the rights
of patients and ensure that the quality of care they receive meets all state
and federal requirements. Provide coordinated discharge planning services
focused on continuity of care, patient and family preferences and available
resources.  Supports and participates in the Core Vales and Mission of the
Providence Health System.

ESSENTIAL JOB FUNCTIONS:

duties of this job)

A.      JOB DUTIES: (For performance review, assess competence for each
essential function using “C” for competent and “NI” for needs
improvement)
       
1.      Manage a caseload and maintain an organized, current case file
system that reflects the psychosocial needs of the patient and the efforts
made to meet those needs.
       
2.      Record social work interventions, observations, assessments, and
other important data, in the medical record.

3.      Completes initial social intake and assessment history for each
patient at admission.  Ensures that the patientsÂ’ comprehensive care plan
includes interventions that address social services needs.

4.      Assist new patients and their families in adjusting to cancer
treatment in the outpatient setting.

5.      Assess and identify the medically related psychosocial needs of
patients and assist in developing a plan of care.
       
6.      Participate in multi-disciplinary care conferences and intra-agency
meetings to facilitate treatment, needs assessment, and discharge planning,
       
7.      Arrange for and coordinate the transfer of appropriate patients to
specialty cancer centers amd/or other community placements, and assure
continuity of care.
       
8.      Meet the psychosocial needs of patients by providing crisis
intervention services, referral to community resources and pertinent
information as needed or directed.
       
9.      Provide training, consultation, and assistance to facility
personnel, as needed or requested.
       
10.     Maintain and enhance professional competence and personal growth
through on-going education, in-service training, workshops and seminars.
       
11.     Accept and integrate social work supervision as arranged or provided
by the facility.
       
12.     Perform other duties and responsibilities as assigned.
       
13.     Adhere to the professional guidelines and principles of NASW
standards.
       
14.     Maintain appropriate standards of dress, professional behavior, and
ethical conduct when functioning as a representative of the facility.

15.     Implement care/services that recognize age/diversity specific
needs/issues of customers served.

16.     Performs other related duties as required.

D.      ESSENTIAL JOB QUALIFICATIONS: (Any equivalent Combination of
Knowledge, Skills, Abilities, Education, and Experience)

1.      Education:  A B S W required.  M S W degree in Social Work
preferred.

2.      Experience:  One year of recent experience in a medical facility.

3.      Licensure/Certification: Must meet all state licensing requirements.
       
4.      Other Qualifications: The responsibilities for this position
generally require a 40-hour workweek.  However, additional time may be
necessary due to special events, crisis situations and/or unexpected demands
of the caseload. Ability to effectively communicate in English, both orally
and in writing. Ability to assess psychosocial behavior and medically
related patient needs, and develop a meaningful plan of care, appropriate
interventions, and effective discharge plans. Ability to effectively
communicate with patients, families, other professionals, and the community,
in a positive, professional manner. Ability to articulately express in
writing, social work interventions, patient assessments, plans of care, and
place these in the record in a timely manner as required by facility policy.
       
5.      Attendance:  Regular attendance is a requirement of this position.
       
6.      English Language: Must be able to read, write, and speak English.
