To: CMS RO 10 Medical Provider and Partner Associations:
The number one reason for Medicare provider enrollment processing delays is
the failure by individuals and organizations to submit a complete enrollment
package. The attached MLN Matters articles stress the importance of
submitting a complete enrollment application and all supporting
documentation, including the submission of the National Provider Identifier
and the CMS-588, Authorization Agreement for Electronic Funds Transfer, at
the time of filing. In addition, the Medicare Provider Enrollment web site:

facilitate their enrollment into the Medicare program (see the download
